∙I understand that this service by SEED Academy is for career guidance only and is not a substitute for mental health counselling, psychotherapy, care or diagnosis or treatment of any other mental or physical ailment.
∙I understand that career guidance is not to be used as a substitute for professional advice by legal, medical, financial, business or other qualified professionals.
∙I understand that I am solely responsible for any decisions or actions that I take, even if based on the information or discussion in the course of these sessions.
∙I am responsible for my own privacy in my environment during these online sessions.
∙I give consent to the recording of the sessions YES.
∙I consent to notes being taken during the sessions.
∙I acknowledge that while SEED Academy and will make every effort to maintain the confidentiality of our discussion, they may be required to disclose information in certain legal or ethical circumstances
∙I understand that SEED Academy may not continue with a session or future sessions at their discretion and If a paid session cannot be fulfilled by the Academy, the fees will be refunded. ​
∙If I cancel a session with less than 24 hours’ notice, I can reschedule the session and if not, will lose my session fees.
